Under Section I

Select the following from the list provided.

"Hospital", "Department", "Location / Unit"

Under Section II

Select 'Nickname' of the BGA used for analysing the EQAP sample.

Verify model and serial number given.

Under Section III

Enter result of 'Sample 1' and 'Sample 2' into corresponding space.

Enter feedback as required.

Click "Submit Result" and "Yes" to confirm.

Read 'System Message' to confirm successful submission.

Click the link "Testing Glucometer Form" to print report.
