Alkaline Phosphatase (ALP) Isoenzymes Electrophoresis, Serum

Specimen requirement

Adult patient:

Clotted blood tube (Serum gel separator clot activator tube)

Minimum blood volume: 5 mL

Paediatric patient:

Paediatric clotted blood tube

Number of vials: 2

Minimum blood volume: 1.6 mL

Authorisation code required No

Method Alkaline phosphatase isoenzymes electrophoresis
Reference interval

Not applicable

See result interpretation session.

Clinical indication

When the total ALP activity is increased, the isoenzymes are used in determining the source of the increased activity.

Result interpretation Elevation of total ALP activity with normal GGT activity suggests that majority of ALP is of bone origin.

Elevation of total ALP activity with increased GGT activity suggests that majority of ALP is of liver origin.

If non-bone, non-liver ALP is suspected, ALP isoenzymes electrophoresis helps confirm origin(s) of ALP.
